Marching Band Only Registration

In consideration of being permitted by Tampa Bay Pride Band, Inc. to participate in activities at any of its rehearsal, practice, performance, and/or social facilities, I hereby waive, release and discharge any and all claims for damage for personal injury, death or property damage which I may have, or which may hereafter accrue to me, as a result of participation in activities at said facilities. This release is intended to discharge in advance the Tampa Bay Pride Band, Inc. and all of its performing ensembles, its officers, employees and agents from any and all liability arising out of or connected in any way with my participation in activities at this or any other Tampa Bay Pride Band, Inc. event even though that liability may arise out of negligence or carelessness on the part of those parties. It is further agreed that this waiver, release and assumption of risk is to be binding on my heirs and assigns. I agree to indemnify and to hold harmless, the Tampa Bay Pride Band, Inc., its officers, employees and agents from any loss, liability, damage, cost or expense which they may incur as the result of my death or any injury or property damage that I may sustain while participating in any Tampa Bay Pride Band, Inc. activity at any rehearsal, practice, performance, and/or social facility. I understand that by participating all Participants consent to photo images taken by the Tampa Bay Pride Band, Inc. staff or members during my participation to be used in any or all Tampa Bay Pride Band, Inc. publications and websites.

I the undersigned, and my personal representatives, assigns, successors, heirs, and executors, shall indemnify and save harmless the Tampa Bay Pride Band and Tampa Bay Symphonic Winds, its members, officials, officers, employees, agents, affiliates, and volunteers from any and all claims, demands, suits, actions, judgments, and executions for damages of all kinds, losses, costs or expenses of every nature whatsoever caused by the negligent acts or omissions of myself at the Event, or caused by any negligent act or omission of myself. The undersigned further hereby assumes full responsibility for any and all risk of bodily injury, death or property damage while participating in the Event. The undersigned further expressly agrees that this document is intended to be as broad and inclusive as is permitted by Florida law and that if any portion hereof is held invalid, it is agreed that the balance shall; notwithstanding, continue in full legal force and effect. The undersigned has read and fully understands this Release, Waiver of Liability and Indemnification and voluntarily agrees to its terms. Further, all terms of this agreement are contained herein and there are no other terms to this agreement.
